Receiving a diagnosis of a life-limiting illness can create moments of sadness, fear, and many questions. At the same time, families must try to navigate the health care system and understand what they can expect to come. Our Palliative Care Community Team has specially trained staff who will be with you and your family from early diagnosis, through bereavement. We will work with other members of your care team and are available to help you and your family throughout this journey.
If you have been diagnosed with a life-limiting illness we encourage you to get in touch with our team as soon as possible so we can start supporting your goals of care.
The Palliative Care Community Team will:
- Support you and your family in your own home
- Help connect you to community services and health care providers
- Provide expert palliative consultation to you and members of your care team
- Link you to 24/7 on-call physician coverage to support you and your family
- Provide psychosocial support to you and your family
